Filling Out WORD Forms On Line

PLEASE NOTE:

• Our forms are in MS WORD format and are available to be filled out on-line, or simply printed and filled out manually.
• In order to fill out our WORD forms on-line, your browser must be configured to open (not just view) MS WORD. 

Hints for completion, on-line.

• When you open a form, the first fill-in area will have a darker gray highlight, just begin typing.
• Use the TAB key OR arrow keys to advance from field to field.
  
• You may click the space bar OR the mouse button to insert or remove an "x" in the check boxes.
  
• You may wish to view your document at 100% in order to see more clearly.
  
• Each fill-in area has a limited amount of space, you may need to attach and clearly cross reference your attachment.
  
• Print and save these documents as you would any WORD document--be sure to use the SAVE AS feature and save to your own drive, otherwise the file could end up in your "temp" directory.
